Patient Forms
To help make your appointment as smooth and efficient as possible, we encourage you to complete your patient forms before arriving at our office. Simply download the appropriate forms below, fill them out at your convenience, and bring the completed copies with you to your appointment. We offer both our Patient Intake Forms and HIPAA Forms in English and Español for your convenience.
What to Expect at Your First Appointment
When you visit us for your hearing healthcare, you will always be greeted by Jaysee or Grace. Your appointment will be broken into four parts.
1. Consultation: We will have a conversation to get to know you. We will ask you some questions about yourself, your hearing, medical history, and family history.
2. Ear Exam: We will take a thorough look at the inside of your ears using an otoscope. We want to ensure impacted earwax is not affecting your hearing before moving on to the hearing test.
3. Hearing Test: A complete hearing evaluation will be performed to assess your ability to hear different sounds and frequencies.
4. Results: We will go over the results of your hearing test with you which are laid out on your audiogram. Any questions you have will be answered so you feel informed about your hearing health.
