Information you provide

When you contact us or request an appointment, you may provide your name, phone number, email address and information about your request. We use the information you provide to respond to your questions and help with appointment requests. Website inquiries are delivered to our front-office team through email and text notifications.

Our contact form uses Basin to receive and store form submissions. The form asks for your name, phone number, email address and message. Information you submit is processed through Basin so we can manage your inquiry.

For questions about sending medical or insurance information, please call our office so we can direct you to the appropriate way to share it.

Appointment requests and patient forms

Our online scheduling tool is provided through MyHearingPortal. Information you enter in that tool is handled through its scheduling service to process your appointment request.

Our patient-forms page provides downloadable PDFs and instructions for returning completed forms by fax or bringing them to your visit.

Website analytics

We use Plausible Analytics to understand how people use our website and improve its content. Its reports summarize information such as pages visited, referral sources, browser and device types, approximate location and engagement with the site.

Plausible does not use analytics cookies or persistent visitor identifiers, and it does not track visitors across websites or devices. It processes an IP address and browser information to calculate statistics without storing the raw IP address or browser user-agent string. We do not currently use advertising pixels or call-tracking services on this website.

Maps, videos and external links

We display Google Maps to help you find our office and YouTube videos to share hearing-care information. When these features load or you use them, your browser connects to Google. Google may receive technical information, such as your IP address and browser details, and information about your interaction with its services. Its use of cookies and similar technologies is described in Google’s Privacy Policy.
